TRACKING Logs FOR Agency CONTRACTORS

Each tracking log comes with a rate sheet to be filled in for hours, units or visits.  There are separate logs for OT,  PT and ST.  The OT and PT logs will also allow you to bill separate amounts for COTA's and PTA's.  

Whether you're serving hospitals, nursing homes, home health agencies, or school districts you'll be able to keep an accurate count of visits, units or hours spent treating patients.  This tool will track multiple therapists or multiple clients for each contract. Here's how it works:

Suppose you contract with Sunny Vista Care Center.  Your contract states you're reimbursed hourly. You will put in your hourly reimbursed rate on the rate sheet.

You record the names of your therapists working at Sunny Vista.  Then list the names of the Sunny Vista residents receiving speech therapy that month. (There is room to list 5 therapists and 37 residents per month on each page).  The therapists then record the number of hours worked each day.  Since Sunny Vista bills Medicare in units, your therapists record the number of therapy units completed for each session as well.  Right now we have Joe and Susie providing therapy to Alice and Dan.

Our log will compute the total hours worked AND the productivity of your therapists!  Sunny Vista will probably expect your therapists to be 70-80% productive. You can now show your administrator productivity daily!!  So far Joe and Susie are being 55.36% productive and have done 31 units of billable therapy. 

Finally, our tacking log will also total your charges at Sunny Vista.  In our example, you bill Sunny Vista $50 an hour for therapist time on premise.  That would total $700 for the 14 hours Joe and Susie have worked.
